Özet

Bu makalede; trombositten zengin fibrinin yara sağaltımındaki önemi vurgulanmıştır. Yara iyileşme süreci yara bölgesine salgılanan çeşitli sitokinler ve büyüme faktörleri tarafından sıkı bir şekilde düzenlenir. Trombositler, yara iyileşmesinde merkezi bir rol oynayan hücrelerdir ve yara iyileşmesinde ana rolü oynayan büyüme faktörü kompleksinin ana kaynağıdırlar. Yara bölgesine çok hızlı ulaşırlar ve pıhtılaşmayı başlatırlar. Trombositler aktive edildiğinde, büyüme faktörlerini, sitokin ve kemokin salgılarlar. Bu faktörlerin etkileşimi anjiyogeneze ve doku rejenerasyonuna yol açar ve yaranın iyileşmesini sağlar. Trombositten zengin fibrinin yara iyileşmesinde kullanılmaya başlamasıyla birlikte daha iyi sonuçlar almak için süreç içerisinde jel formları (Trombositten zengin fibrin (TZF) veya Lökosit ve trombositten zengin fibrin (L-TZF), gelişmiş trombositten zengin fibrin (G-TZF), Yatay santrifüjleme ile elde edilen trombositten zengin fibrin (Y-TZF) ve Titanyumla hazırlanmış trombositten zengin fibrin (T-TZF)) ve sıvı formu (Enjekte edilebilir trombositten zengin fibrin (E-TZF)) kullanılmıştır. Uygulanacak bölgenin anatomik durumu bu uygulama formlarının çeşitliliğine neden olmuştur. Bu nedenle trombositten zengin fibrin formlarının hangisinin kullanılacağına yaranın bulunduğu bölge ve yaranın özelliği göz önüne alınarak karar verilmelidir. Elde edilmesi kolay olan trombositten zengin fibrinin yara sağaltımında kullanımının gün geçtikçe yaygınlaşacağı düşünülmektedir.
